BAYADA is helping to build a new Medication Management company and is looking for a Medication Manager to expand the program into Cumberland County. This is a hands:on, fast paced environment, where the Medication Manager will have to roll their sleeves up to help build an integrated Medication Management program into BAYADA. You will report to the Director of Medication Management but will have great visibility to see all aspects of the operation and the team itself. This person will perform in home visits and calls to prospective clients recommended by the BAYADA Team to present and transition the client to the Pharmacy turn, the Medication Manager is responsible for providing efficient day:to:day coordination, delivery and internal marketing of pharmacy services. You will perform client visits, pharmacy transfers, implement and facilitate pharmacy marketing activities and resolve customer related issues.Learn all aspects of our integrated medication management system, including:The CarePack system (box, shipping, forms, transfer) to others. Obtain, manage and follow:up on client transfers assuring optimal use of our partner Pharmacy. Accurately identify medications and their actions to help clients manage them more efficiently. Available resource/educator in identification of pharmacy candidates and fulfillment of client needs. Make home visits to clients in designated geographic municate daily with referral sources Client Service Managers, Home Health nurses, Clinical Managers, Directors and Physicians.Assist in the development, implementation and evaluation of marketing objectives to internal resources, using creativity, initiative and ingenuity to achieve business growth goals. Ability to create and execute professional presentations in various settings, including but not limited to; medical clinics, senior centers, client homes and support groups. Perform basic nursing care and treatments as neededObserve, document and report any changes or concerns in client condition to the appropriate personnel and complete mandatory reporting, A?hot:liningA? , when appropriate, asA?required by law. Comply with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patientA?s care. As business needs may change, may perform additional responsibilities assigned by the Director that are reasonably related to the position.A: Licensed RN in good standing in the state of New Jersey.A: A minimum of one year, recent and verifiable clinical experience in the home.A: Excellent verbal and written communication skillsA: Demonstrated record of strong interpersonal skills and relationship buildingA: Exposure to sales in some capacity is not necessary, though helpfulA: Demonstrated ability to read, write, and effectively communicate in English. Bilingual English /Spanish preferred. A: Ability to work independently and manage time effectively.A: Strong interpersonal, organizational, and problem solving skills.A: Competence in basic PC skills required to perform job functions.
